If you are a new patient please read the following: Please complete the "Patient Intake Form" and the "Credit Card Authorization Form" (please completely fill out all pages and sign). Bring these with you for the first appointment. The office is located off the center courtyard in the back of the complex, park in the Southwest end of the parking lot to enter the Center courtyard.
